A Social Security employee, Dan R. Hyatt , has stolen my client’s social security number and other private medical information and has broadcast it across state lines, according to reports by the Oregon State Bar and OSB complaints.

Thereafter, my social security number, attached to a SSA 1695 form turned up missing at the same office where Dan R. Hyatt is employed.

I have filed a complaint with the Oregon State Bar requesting that they investigate to see if there is any theft and sale of social security numbers. There are

10,000 social security numbers accessible just outside Dan R. Hyatt ’s door,


he has handled 6,600 social security numbers personally and


has access on the SSA computer program CPMS to the millions of social security numbers of citizens through the United States, her territories, possessions and Americans living overseas.

Therefore, please provide a free copy of my credit report to see how much damage the loss of my social security number has caused me.

I fear that once word gets out about Dan R. Hyatt ’s promiscuous broadcasting of Social Security number that you may get several hundred thousand or millions of other requests for free credit reports. I suggest you contact the Chief Regional Judge of Social Security and the Commissioner of Social Security Astrue on how to handle this future demand upon your resources.
